For me the easiest is to lasso around the eyes and teeth, put them on a new layer, use the curves to brighten them up and desaturate if needed. Then I add a mask to the top (original) layer and brush away til the brighter/whiter eyes and teeth layer comes through. Takes just a few minutes. The nice thing about this method is that you can reduce the opacity of the brightened teeth/eyes layer until it's just right at whatever image size you will be using it.
